What Makes Our Primary Care Different

Root-cause, lifestyle-focused care: CFMA providers look beyond symptoms to understand the daily habits, nutrition, sleep, stress, and metabolic factors that impact your long-term health.

Metabolic health and weight expertise: As a board-certified Obesity Medicine physician, Dr. Reller and her team bring advanced training in insulin resistance, metabolic dysfunction, and weight management. Our structured Weight Management program is a membership-based service with additional fees and its own visit structure, separate from standard primary care. Data-driven decisions: We use body composition analysis, metabolic assessments, continuous glucose monitoring (CGM), and advanced preventive labs to detect problems earlier and personalize your plan.

Integrated chronic disease support: For qualifying patients, we offer Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) to support blood pressure, glucose, weight, and sleep between visits.

Convenient in-house testing: Many tests can be done right here, including ECG, PFT, body composition, and rapid testing. Bloodwork is not drawn in our office; instead, we utilize outside laboratories that work with your insurance or provide cash-pay options.

Care for teens, adults, and seniors: We provide primary care for patients ages 10 and up. Younger children can still be seen in our allergy, asthma, and immunology clinic when appropriate, but we do not provide general primary care for children under 10.

Thoughtful scheduling: We frequently have same-day or urgent appointments available. We highly recommend calling the office first so we can fit you into the most appropriate time slot rather than walking in.

Insurance and membership options: We participate with many major insurance plans. For patients without insurance or with high-deductible plans, we also offer a Direct Primary Care membership for more predictable costs and improved access.

We order a wide range of preventive and diagnostic labs. Bloodwork is performed at outside laboratories that work with your insurance whenever possible, or via cash-pay lab options when needed.

Vaccines

We currently carry:

  • Prevnar 13
  • Pneumovax 23
  • Tetanus vaccines

We do not stock most routine vaccines—such as Meningococcal, MMR, Shingles, Varicella, HPV, Hepatitis A/B, or DTaP/Tdap—but we can refer you to a local pharmacy.

For pediatric patients, please ensure your child is up to date with the CDC-recommended vaccine schedule.

Frequently Asked Questions

Do you take insurance?

Yes. We accept many major insurance plans for primary care services. We also offer a Direct Primary Care membership for patients without insurance or with high-deductible plans who prefer predictable monthly costs and simplified access.

Do you see children?

We provide primary care for ages 10 and older. Younger children may be seen in our allergy, asthma, and immunology clinic for those specific concerns.

What should I bring to my first visit?

Please bring your insurance card, a list of your current medications and supplements, and any recent labs or imaging. We also ask that you complete the new patient paperwork emailed to you and return it before your appointment whenever possible.

Do you offer telemedicine?

Yes. Telemedicine is available for follow-ups, medication check-ins, and certain concerns when appropriate and allowed by your insurance.

Do you offer same-day or urgent visits?

Often yes, depending on availability. Please call the office first so we can schedule appropriately. We strongly prefer a call rather than walking in.

Can you help with weight loss and metabolic health?

Yes. We address metabolic health during primary care visits. For patients wanting structured support, we offer a membership-only Weight Management program with additional fees. See our Weight Management Membership page for details.

Do you refill medications?

Do you do sports physicals and pre-op evaluations?

Yes. Please let our staff know what forms or requirements are needed when scheduling.
