NEW PATIENTS: All new patients will need to bring a photo ID, their medication lists, and insurance cards to their first appointment. Please arrive 30 min prior to your appointment to allow time to fill out paperwork. You may also download the forms from our website if applicable.
Co-Pays and deductibles are collected before the appointment.
All accounts must be kept current and with zero balance in order to continue to receive care. Please contact the office manager for any questions with your account.
REFILLS: Please allow our office 48 hours notice for prescription refills. There will be no refills on medications if the patient has not been seen within the last 12 months or is overdue for a follow up visit. If you are due for an appointment, only a one month supply may be given.
REFERRALS: We require a 72 hour notice for all referrals. Referrals take a period of time to be processed by us and your insurance company. Please keep this in mind when making appointments. A referral request without a 72-hour notice will require your appointment to be rescheduled. Please make sure your referrals are in place if you have an appointment for a specialist visit.
LAB RESULTS: All lab results obtained through the patient portal (Quest and Labcorp only) are for your viewing. If you have any questions pertaining to your labs, you will need to discuss the results on your next appointment with the doctor. Office staff cannot answer your questions.
NARCOTIC PRESCRIPTIONS: We have a no tolerance policy for narcotic misuse. All schedule II substance visits must be in person according to state law. Each schedule II prescription will be for no more than 30 days and no refills. A signed narcotics agreement must be on file. Urine drug screen may be requested at any time. Other controlled drugs require in-person visits at least every 3 months but may be managed by telemedicine according to state and federal laws.
Please refrain from wearing perfumes and body sprays. Our patients as well as our doctors and office staff have allergies and/or asthma as well as migraines and may have severe reactions to these odors. This notice is also listed on all our entry doors.
The trend of the family physician admitting patients to the hospital is no longer common. If you need hospital admission, we will do our best to coordinate your care at Morton Plant Hospital, Countryside Mease Hospital, or Advent Health Tarpon Springs and you will be admitted to their respective hospitalist teams. For family medicine, we do not have hospital privileges but can work closely with the hospitalist, including sharing your medical record, to ensure you receive the best care.
