CFMA Basic Membership Pricing

Choose from a one-year (52 weeks) or six-month (26 weeks) plan. Billed every 4 weeks.

One-Year Plan

Ages 16–20: $35 every 4 weeks
Ages 21–40: $65 every 4 weeks
Ages 41–60: $85 every 4 weeks
Ages 61+: $110 every 4 weeks

Six-Month Plan

Ages 16–20: $45 every 4 weeks
Ages 21–40: $75 every 4 weeks
Ages 41–60: $95 every 4 weeks
Ages 61+: $120 every 4 weeks

How Do I Get Started?

We are currently accepting patients and adding to a waiting list. Membership starts January 1, and availability is limited. Don’t miss your chance — secure your spot early!

Do I Need Insurance?

With the membership, CFMA does not bill insurance and insurance is not required. However, it is recommended to maintain insurance coverage to help pay for labs, imaging, medications, and hospital services. CFMA is not responsible for outside service costs.

HSA/FSA: Our services may qualify for reimbursement through your HSA or FSA. Contact your benefits administrator to confirm eligibility. Medicare does not cover membership fees, and most private plans do not reimburse them.

What if I Have an HMO or Medicaid?

Patients with an HMO plan through commercial insurance or Medicaid (primary or secondary) are not eligible for the CFMA membership program.
