Coverage note: We make every effort to use insurance when possible. Continuous glucose monitors (CGMs) may not be covered for patients without diabetes, and TrioSmart SIBO testing may not be covered by all plans. We will review any potential out-of-pocket costs before ordering.

Our Treatment Philosophy

Chronic disease care works best when medications, lifestyle and monitoring all support each other. We are comfortable starting medication when needed to get things under control, and just as comfortable working to reduce or stop medications when your health allows.

Step 1

Stabilize and Help You Feel Better

We begin with a detailed history, exam and review of prior records. If your blood pressure, blood sugar, sleep or symptoms are unsafe or significantly impacting your life, we start appropriate medications and short-term treatments while we gather more data.

Step 2

Find Root Causes and Build a Plan

Using advanced labs, tools like CGM, sleep testing, TrioSmart and body composition analysis, we look for the “why” behind your condition. Then we design practical changes in nutrition, movement, sleep, stress and habits that fit your real life.

Step 3

Support, Adjust and Deprescribe When We Can

Over time, we adjust medications, monitor progress and, when it is safe, work toward lowering doses or stopping medicines altogether. We stay in touch through follow-up visits and, when appropriate, programs like health coaching, CCM and RPM.

What to Expect at Your Visit

Whether you're new to our practice or checking in for ongoing chronic disease care, we want you to feel comfortable, informed, and supported. Here’s what your visit typically includes:

Step 1

Review of Your Concerns and Symptoms

We begin by understanding your day-to-day symptoms, medical and family history, lifestyle, stressors, and what you’ve already tried. You’ll have time to share your concerns without feeling rushed.

Step 2

Thorough Evaluation & Examination

We review blood pressure, weight trends, medications, and any prior labs or imaging. When needed, we order advanced labs, CGM, sleep testing, GI testing, or body composition to get a full picture.

Step 3

A Personalized, Actionable Plan

Together we create a plan that may include medication, lifestyle adjustments, nutrition changes, stress management, supplements, referrals, and follow-up testing.

Step 4

Follow-Up & Long-Term Support

We check in regularly to track progress, adjust your plan, and—when appropriate—reduce or stop medications. CCM, RPM, and in-office health coaching provide extra support between visits when needed.

Chronic Disease Management FAQs

Here are answers to some of the most common questions we hear about chronic disease care, testing, and follow-up at our office.

Do I have to already have a diagnosis to be seen?

No. Many patients come to us with symptoms such as fatigue, weight changes, bloating, headaches, blood pressure concerns, or abnormal labs and are not sure what is going on yet. We can help with both diagnosing and managing chronic conditions.

Are continuous glucose monitors (CGMs) covered by insurance?

Coverage varies by plan. CGMs are more commonly covered for patients with Type 1 or Type 2 diabetes. They may not be covered for patients without a diabetes diagnosis. If a CGM could be helpful for your care, we will review your insurance coverage and discuss any potential out-of-pocket costs before moving forward.

Is the TrioSmart SIBO breath test covered by insurance?

Some insurance plans do not cover TrioSmart testing or may cover only part of the cost. Because coverage is variable, we will talk through pricing and options ahead of time so you can make an informed decision.

Can chronic disease visits be done by telemedicine?

Certain follow-up visits and medication checks may be appropriate for telemedicine, depending on your condition and insurance coverage. Some visits still require in-person evaluation, physical exam, or testing. Our team will help you choose the best format for each visit.

How often will I need follow-up visits?

Follow-up frequency depends on the conditions we are managing and how stable things are. Early on, visits may be more frequent while we are adjusting medications and working on lifestyle changes. Once you are stable, visits are typically spaced out but still frequent enough to prevent things from slipping backward.

Do you work with my other specialists?

Yes. We regularly coordinate with cardiology, gastroenterology, neurology, sleep medicine, endocrinology and other specialists. Our goal is to make sure everyone is on the same page and that you have a clear, connected plan.

Can you help me come off of medications?

In many cases, yes—if it is safe. We cannot promise that every medication can be stopped, but improving sleep, nutrition, movement, stress and metabolic health often allows us to reduce doses or deprescribe certain medications over time. Any changes are made gradually and with close monitoring.
